The boss of a small yakuza gang called Medaka-gumi, who has only four underlings, dies leaving a will stating that his successor must be a blood relative. Around the same time, high school girl Izumi Hoshi is saying her final goodbye at the crematorium to her father Takashi, who died after being hit by a car in front of Narita Airport. Izumi's mother passed away long ago, leaving her truly alone. When Izumi returns to her apartment, a woman named Mayumi is there, carrying a letter from her father that says, 'If I die, please take care of Izumi.' The next day, a crowd of men in black suits lines up in front of her school. Ignoring her friends Tomoki, Tetsuo, and Shuhei, who try to stop her, Izumi heads toward the school gate. The man from the crematorium steps forward and says, 'You're Izumi Hoshi, right?' This man, named Sakuma, takes her to a shabby office and asks her to succeed as the fourth-generation boss of the Medaka-gumi.
